1. For accidental injuries confirmed to be work-related, clearly inform the attending physician at the time of consultation. Work injury identification application shall be submitted within 30 days after the injury. Applications submitted beyond the time limit will not be reimbursed by work injury insurance, and all relevant medical expenses shall be borne by the employer. All medical expenses incurred prior to work injury identification shall be settled at personal expense first. After the work injury is officially recognized, the patient may return to the hospital to apply for retroactive work injury billing.

2. Documents Required for Provincial Work Injury Retroactive Billing

(1) Medical invoices for outpatient and inpatient services

(2) Three copies of both sides of the injured person’s ID card

(3) Three copies of both sides of the agent’s ID card

(4) Two copies of the Work Injury Identification Certificate

(5) If the refund needs to be transferred to the corporate account, the employer shall provide one letter of authorization affixed with the official company seal, signed and fingerprinted by the injured person.

3. Important Notes

The injured person, family members and employer representatives shall truthfully describe the accident process and provide authentic medical records. Submission of false documents is prohibited, and violators will be penalized for insurance fraud by higher authorities.
