Founded in 1997, Luotian Community Health Service Center is equipped with General Practice, Gynecology, Stomatology Dept, Ultrasound Room, Clinical Lab, Pediatric Healthcare Room and Immunization Registration & Vaccination Room. The center has a total of 24 staff members: 6 general practitioners, 2 gynecologists, 2 public health physicians, 1 TCM physician, 7 community nurses, 1 lab technician, 1 ultrasound physician, 2 pharmacists and 2 supporting staff. It provides primary medical care and public health services for over 70,000 residents in Luotian Community, Yanluo Subdistrict.
2. Service Scope
2.1 Clinical Services
The center delivers diagnosis and treatment in general practice, gynecology, stomatology and traditional Chinese medicine. It manages various common community diseases, performs minor general surgical and routine gynecological procedures, and provides rehabilitation care for patients with severe illnesses.
2.2 Public Health Services
1. Personal Health File Establishment: Create individual health records and deliver continuous health management services for residents.
2. Health Education: Carry out diversified health education activities to improve residents’ health literacy.
3. Immunization Services: Provide routine vaccination for children aged 0–6 in accordance with immunization schedules, and offer selected vaccines for adults.
4. Pediatric Healthcare: Offer standardized physical examinations and health guidance for children aged 0–3 within the service coverage area.
5. Maternal Health Management: Conduct early pregnancy screening for women of childbearing age and full-cycle health management for pregnant and postpartum women.
6. Elderly Health Management: Provide annual free physical examinations and regular health management for residents aged 65 and above.
7. Health Management for Hypertensive and Type 2 Diabetic Patients: Arrange one free physical examination per year; patients with Type 2 diabetes can receive four free fingertip blood glucose tests annually.
8. Health Management for Patients with Severe Mental Disorders: Deliver regular health management, assist community service stations in handling emergencies involving mentally ill patients, and arrange one free physical examination each year.
9. Tuberculosis Patient Management: Screen suspected tuberculosis cases and arrange referrals; implement long-term health management for confirmed tuberculosis patients in the community.
10. TCM Health Management Services: Supply multiple appropriate TCM therapies, as well as special TCM health services for children aged 0–36 months and seniors over 65 years old.
11. Infectious Disease Prevention and Control: Timely report infectious diseases detected in daily diagnosis and treatment per regulations, and assist disease control departments in handling public health emergencies.
12. Active Surveillance of Foodborne Diseases: Promptly report foodborne diseases found in routine medical work as required.
13. Free Contraceptive Distribution Service: Set up self-service and manual distribution counters to provide free contraceptives and relevant consultation for residents.
14. HIV Prevention and Control: Popularize HIV prevention knowledge, and offer HIV-related consultation and referral services.
2.3 Family Doctor Contracted Services
The center provides a full set of public health and basic medical services including contract management, health file creation, health assessment, intervention, tracking and follow-up visits. On top of basic medical care, contracted residents can access personalized proactive health management services, including early health assessment, timely health information notification, categorized initiative care and thoughtful home-based services.
2.4 On-site Specialist Clinics
Dermatology specialists Chief Physician Zhang Yanjie and Associate Chief Physician Fan Kaiwen hold regular clinics every week. They can accurately diagnose and treat complicated dermatological disorders, allergic diseases (urticaria, eczema, drug eruption), facial acne and fungal infections, and possess abundant clinical experience in diagnosing and treating sexually transmitted diseases such as condyloma acuminatum, genital herpes, gonorrhea, non-gonococcal urethritis and syphilis.
Associate Chief Physician Wen Zhengwei, specialist in gastroenterology and hepatology, attends clinics weekly. He excels in standardized diagnosis and treatment of digestive and liver diseases, and has rich experience in rescuing severe cases in gastroenterology and hepatology. He is proficient in endoscopic operations, liver ultrasound imaging and transient elastography.

(1) Characterized Pediatric Healthcare Model
In recent years, the center has transformed the single traditional service mode into human-centered care. Staff provide gentle care for children receiving physical examinations and vaccinations, strengthen emotional communication, optimize service environment, organize health lectures, issue contact cards and maintain timely communication and feedback with parents. This model shortens the psychological distance between families and medical staff, builds mutual trust, relieves children’s tension, enables smooth vaccination, achieves high parent satisfaction and is worthy of wider promotion.
(2) Enterprise-oriented Health Education Program
A special health education lecture group composed of renowned specialists from the hospital, chronic disease hospital and maternal & child health hospital delivers health services to enterprises. Various activities including thematic lectures, health consultations, health knowledge exhibitions and customized publicity boards are held to disseminate knowledge on stress relief, adolescent women’s health, first aid and infectious disease prevention. Up to now, 103 enterprise lectures have been delivered with over 80,000 brochures distributed, raising employees’ health awareness and self-care ability and winning widespread praise.
(3) Characterized Traditional Chinese Medicine Services
Eight types of appropriate TCM therapies are available for residents: cupping, acupuncture, moxibustion, scraping, tuina, traction and physical therapy. These therapies are widely applied to prevent and treat common, frequent and chronic community illnesses, integrating TCM techniques into primary medical and public health services and gaining great popularity among residents.
