1. Center Overview
Founded in 1998, Yanchuan Community Health Service Center is equipped with General Practice, Pediatric Healthcare Room, Immunization Registration & Vaccination Room and Clinical Lab. It has a total of 17 staff members: 6 general practitioners, 10 community nurses and 1 pharmacist, with no full-time public health physicians. The center undertakes basic national public health services for Yanchuan Community and meets the medical treatment demands of surrounding residents.
2. Service Scope
2.1 Clinical Services
The center provides diagnosis and treatment in general practice and traditional Chinese medicine. It treats a full range of common community diseases, performs minor general surgical procedures, and delivers rehabilitation care for patients suffering from severe illnesses.
2.2 Public Health Services
1. Personal Health File Establishment: Create individual health records and deliver continuous health management services for residents.
2. Health Education: Carry out diversified health education campaigns to improve residents’ health literacy.
3. Immunization Services: Provide routine vaccination for children aged 0–6 in accordance with national immunization schedules, and offer selected vaccines for adults.
4. Pediatric Healthcare: Deliver standardized physical examinations and health guidance for children aged 0–3 within the service coverage area.
5. Maternal Health Management: Conduct early pregnancy screening for women of childbearing age and full-cycle health management for pregnant and postpartum women.
6. Elderly Health Management: Provide annual free physical examinations and regular health management for residents aged 65 and above.
7. Health Management for Hypertensive and Type 2 Diabetic Patients: One free comprehensive physical examination is offered each year; patients with Type 2 diabetes receive four free fingertip blood glucose tests annually.
8. Health Management for Patients with Severe Mental Disorders: Provide routine health management, assist community service stations in handling emergency incidents involving mentally ill patients, and arrange one free physical examination per year.
9. Tuberculosis Patient Management: Screen suspected tuberculosis cases and arrange medical referrals; implement long-term standardized health management for confirmed tuberculosis patients in the community.
10. TCM Health Management Services: Provide various appropriate traditional Chinese medicine therapies, plus targeted TCM health services for children aged 0–36 months and seniors over 65 years old.
11. Infectious Disease Prevention & Control: Timely report infectious diseases identified in daily clinical work in accordance with regulatory requirements, and assist disease control institutions in responding to public health emergencies.
12. Active Surveillance of Foodborne Diseases: Promptly report all foodborne diseases detected during routine diagnosis and treatment as required.
13. Free Contraceptive Supply Service: Equipped with self-service and manual distribution counters to provide free contraceptives and supporting consultation services for residents.
14. HIV Prevention & Control: Disseminate HIV prevention knowledge to community residents, and provide HIV-related consultation and referral services.
2.3 Family Doctor Contracted Services
The center offers a complete package of public health and basic medical services including service contract signing, health file creation, health assessment, intervention, tracking and follow-up visits. Besides basic medical care, contracted residents can access personalized proactive health management services, covering early physical condition assessment, timely health information notification, categorized targeted care and considerate home visiting services.
2.4 On-site Specialist Clinics
An associate chief physician delivers regular outpatient services at the center.Associate Chief Physician Zhang Xueqin has 30 years of clinical experience in pediatrics, specializing in the diagnosis and treatment of pediatric critical illnesses as well as common and frequently-occurring pediatric disorders.
