1. Overview of the Community Health Center
Established on May 18, 2004, Jiyuan Community Health Service Center is equipped with General Practice, Gynecology, Stomatology Dept, Ultrasound Room, Clinical Lab, Pediatric Healthcare Room, and Immunization Registration & Vaccination Room. It has a total of 40 staff members, including 8 general practitioners, 4 gynecologists, 2 stomatologists, 3 public health physicians, 9 community nurses, 8 lab technicians, 1 ultrasound physician, 2 pharmacists and 3 other supporting staff. The center undertakes national basic public health services for Lougang Community and the area east of State Highway 107 in Songtao Community, and meets the clinical treatment demands of local residents.
2. Service Scope
2.1 Clinical Medical Services
The center delivers clinical diagnosis and treatment in general practice, gynecology, stomatology and traditional Chinese medicine. It manages various common community diseases, performs minor general surgical procedures and routine gynecological surgeries, and provides rehabilitation care for patients with severe illnesses.
2.2 Public Health Services
1. Personal Health File Establishment: Create personal health records for residents and deliver continuous health management.
2. Health Education: Conduct diversified health education campaigns to improve residents’ health literacy.
3. Immunization Services: Administer routine vaccinations to children aged 0–6 in accordance with immunization schedules, and provide selected vaccines for adults.
4. Pediatric Healthcare: Deliver standardized physical examinations and health guidance for children aged 0–3 within the service area.
5. Maternal Health Management: Carry out early pregnancy screening for women of childbearing age and offer full-cycle maternal health management.
6. Elderly Health Management: Provide annual free physical examinations and regular health management for seniors aged 65 and above.
7. Health Management for Hypertension and Type 2 Diabetes Patients: Offer annual free physical examinations. Patients with Type 2 Diabetes receive four free fingertip blood glucose tests every year.
8. Health Management for Patients with Severe Mental Disorders: Provide regular health management, assist community service stations in handling emergency incidents involving mentally ill patients, and arrange one free physical examination per year.
9. Tuberculosis Patient Management: Screen suspected tuberculosis cases and arrange referral; implement long-term health management for confirmed tuberculosis patients.
10.Traditional Chinese Medicine (TCM) Health Management: Provide various appropriate TCM therapies, and deliver targeted TCM health services for children aged 0–36 months and seniors over 65.
11. Infectious Disease Prevention & Control: Timely report detected infectious diseases per regulatory requirements and assist disease control authorities in responding to public health emergencies.
12. Active Surveillance of Foodborne Diseases: Promptly report all identified foodborne illnesses as required.
13. Free Contraceptive Supply: Equipped with self-service and manual distribution stations to provide free contraceptives and related consultation.
14. HIV Prevention & Control: Disseminate HIV prevention knowledge and offer HIV-related consultation and referral services.
2.3 Family Doctor Contracted Services
Sign service contracts with residents, build health files, and provide a full package of public health and basic medical services including health assessment, intervention, tracking and follow-up visits. Besides basic medical care, contracted residents can enjoy personalized proactive health management services such as early health status screening, timely health information notification, classified targeted services and home-based considerate care.

2.5 Featured Services
The Gynecology Unit of Jiyuan Community Health Service Center is staffed by experienced associate chief physicians with distinctive expertise in treating common gynecological disorders. It is the only community health service center in Songgang and Yanluo Subdistricts licensed to perform early pregnancy termination surgeries.
