To meet the clinical development demands of the hospital, we plan to conduct market research and selection for partial medical consumables (see Attachment 1 for details). Interested participants shall complete registration via the QR code below before 17:00 on July 9, 2026. Meanwhile, submit the following documents and samples to Office 221, Block C, Huameiju Zone A, Hefeng Road, Bao'an District to complete valid registration: qualification documents of distributors and manufacturers, Power of Attorney (attached with a copy of the authorized representative’s ID card), product authorization letters, Medical Device Registration Certificates, product instructions, and business cooperation certificates (contracts, invoices) between the registering company and other hospitals. All hard copies of the above materials must be affixed with official company seals. Failure to submit paper documents and samples within the specified time limit shall be deemed an automatic waiver of participation.
Notes: Each QR code registration submission only covers one product model. If applying for multiple product models, download the form in Attachment 2 and upload it as required by Question 14 in the questionnaire.
