II. Bidder Qualification Requirements
1. The bidder shall be an independent legal entity or any other organization capable of bearing civil liabilities independently. Provide scanned copies of the business license or public institution legal person certificate; original documents shall be presented for inspection upon request.
2. The bidder shall satisfy the requirements specified in the first paragraph of Article 22 of the Government Procurement Law of the People’s Republic of China, which shall be stated in the Government Procurement Bidding and Performance Commitment Letter.
3. The bidder shall have no major illegal business records within three years prior to this bidding, which shall be stated in the commitment letter.
4. The bidder shall not be under any valid government procurement bidding prohibition issued by competent authorities, which shall be stated in the commitment letter.
5. The bidder shall not be listed in the database of dishonest debtors, major tax violation parties, or serious dishonest entities in government procurement, which shall be stated in the commitment letter.
6. Different suppliers sharing the same legal representative or having direct holding or management relations are not allowed to bid simultaneously. The supplier shall make a signed and sealed statement in the commitment letter.
7. Joint bidding is not accepted. Project subcontracting and transfer are prohibited. The supplier shall make a signed and sealed statement in the commitment letter.
8. This procurement covers Class II medical devices.
For manufacturers: A valid Medical Device Production License is required.
For distributors: A valid Class II Medical Device Business Filing Certificate is required.
9. Submit a valid Medical Device Registration Certificate issued by the National Medical Products Administration for the proposed Class II medical device. Traditional versions shall be accompanied by annexes such as the Medical Device Manufacturing Approval Form or Medical Device Registration Record Form; new version registration certificates do not require annexes. For imported devices, the imported version of the Medical Device Registration Certificate shall be provided.

All copies must bear the company official seal, with the handler’s signature and date. Qualified applicants will receive procurement documents; please bring a USB flash drive for file copying.

VI. Bidding Requirements
Both physical hard-copy bids and electronic scanned bids must be submitted at bid opening. The hospital reserves the right to retain all bidding documents without return after bid opening.

Compile according to the hospital’s bid template. Print double-sided and perfectly bound. Prepare one original and five duplicates (6 sets total), clearly marked accordingly. Duplicates may use scanned versions of the original. In case of any inconsistency, the original document shall prevail.
2. Scanned PDF of the Original Bid
Submit via USB flash drive on-site.
File naming format: BAFY-SBK-2611: Dermatology Treatment Handpieces (Bidder Name)
The bidder shall be fully liable for any losses caused by virus infection or document abnormality.
VII. Other Provisions
Bidders who obtain tender documents but decide not to participate shall notify the Tender Office by phone and in writing before bid opening.
